At this point, you have probably received a "Verbal Offer."

Congratulations!

There is still some work to be done before you arrive which include:

1. Complete/Sign And Send Back (At The Same Time)

Intern Application For Employment
Authorization For Background Check Form

2. Receive, Sign And Send Back The Written Offer Letter

3. Complete The Online On-Boarding process

To make sure you have it all covered, go through the tabs above for the complete process. If you have any questions, just go to the "Contact" page or call/text Coach Mick at the number you were provided.

Completing The Job Application And Background Check Forms

After your verbal offer, you will be sent two documents:

An Application For Employment
An Authorization For Background Check Form

You must fill out both out at the same time and send back together.

Completing The Job Application & Background Check

The first step is to fill out the entire job application and sign it.
You must also fill out the Authorization For Background Check
and sign it.
You must either hand sign both documents or use an electronic signature that you have produced (You cannot use one created for you)
Once complete, you will need to scan and email the completed application and Background Check Authorization to us (unless you used an electronic signature, then just email it back to us)

Once your application is received, and your background check is all set, you will be sent an offer letter.

You must also sign this letter by hand or with an electronic signature that you have produced, not one that was set up for you.

Once complete, you will need to scan and email the offer letter to us (unless you used an electronic signature, then just email it back to us)

On-boarding is where you will officially finalize your intern status.

Once your signed offer letter is received, You will receive an email from Human Resources with your username and password to login to the on-boarding portal which includes:

Direct Deposit Info
EEO Voluntary Disclosure
Employee Handbook Acknowledgement
Federal And State Tax Forms
USTA Confidentiality Agreement Form
USTA Conflict of Interest Form

Once Human Resources has received your on-boarding info, you will receive an email (like the one on the left) that will include:

A reminder of your start date
What time to show up
What to bring with you

* You will need a voided check ( For direct deposit) and two documents supporting your I-9 form which can be:
Drivers License & S.S Card or Drivers License & Birth Certificate or just a Legal Passport
